I understand there is a $150 (normally $297) non-refundable set-up, creation and application fee to cover the development and personalization of my mailers. Also, all mailings will continue as directed unless notified by FAX on or before the 1st of the month (i.e., for April mailer changes, your fax must reach us by March 1st, for May mailer changes, you must reach us by April 1st, etc.).

Fax this form and we’ll send you a Restaurant Birthday Questionnaire. Upon receiving your completed questionnaire, we will send you a proof of our 1st mailer, with belated postcard to follow. Once these pieces are approved, we will assemble and ship your first box of mailings in time for you to place in the mail by the 1st of the month and continue monthly mailers until notified.
